WE NEED YOUR HELP TO MAKE MASSACHUSETTS 2012-2013 PARENT INVOLVEMENT SURVEY A RECORD SUCCESS
Each year the Massachusetts Department of Elementary and Secondary Education (MADESE) surveys parents to determine how well your child’s school is partnering with you and promoting your involvement in your child’s education. All states must collect these data as part of their State Performance Plans required under the Individuals with Disabilities Education Act (IDEA).
The web-based Survey is available now through March 29, 2013.

What is the purpose of the Parent Survey?
¨ The purpose of this survey is to determine how well your child’s school is partnering with you and promoting your involvement in your child’s or guardian’s education.
¨ All states must collect these data as part of their State Performance Plans as required under the Individuals with Disabilities Education Act (IDEA).
Who completes the Parent Survey?
¨ Parents or guardians of children ages 3 to 21 who are receiving special education services through their local school system.
¨ Parents or guardians of children ages 3 to 21 who are receiving special education services who are placed in a nonpublic facility by the child’s local school system.
How will I get this Parent Survey? How do I return it?
¨ The survey can be accessed on the internet at: http://www.maparent1.com.
How do I complete the survey? How can I make sure that my answers are counted?
¨ You may use any computer with an Internet connection to complete the online survey.
¨ If you do not have access to a computer for completing the online survey, your local school or typically public libraries may provide computers with Internet access.
¨ Submit the online survey by March 29, 2013.
Do I have to participate? If I choose not to participate, will it negatively affect my child’s services?
¨ Participation is completely voluntary; however, we encourage you to participate to make your opinions heard.
¨ Your child’s services will not be negatively affected in any way.
I have more than one child who receives special education services. Can I just complete one survey for all of them?
¨ NO. You should complete one survey for each child receiving special education services.
How is the confidentiality of my responses protected?
¨ The survey is confidential. Survey results will only be reported to MADESE in total.
